An additional disability - a medical issue or condition your forgot to file initially that is related to your military service
Another medical condition related to your already established service-connected disability (called secondary condition to your service-connected disability)

Please go page by page through your STR’s and tab each item you wish to claim before meeting with us. Small post-its work great.
Copies of every medical record from every doctor’s office and/or medical center that treated you (including each doctor’s and medical center’s name, address and phone number) for every condition you’re claiming
All medical records must be paper copies, not CD or DVD
